In accordance with Article 6(1)(a) of the GDPR*, as in Art. 81 of 4 February 1994 on copyright and related rights (consolidated text Journal of Laws from 2018, item 1191), I hereby consent to dissemination of my image by the University of Warsaw (the Faculty of Management) in form of a photo in particular by:

• disseminating my image to build the Expert Base of the Faculty of Management, Warsaw University, including the Synergia Portal of the Faculty of Management, Warsaw University: synergia.wz.uw.edu.pl

I hereby waive:

• the fight to control and accept the use of my image each time, including the right to authorise the final version of materials of Warsaw University, in which my image will be used;
• the right to verify each time the personal data of the person shown in the image on synergia.wz.uw.edu.pl, in particular providing their first name and surname;
• Any claims, including remuneration form Warsaw University for using my image for the purposes enumerated in this statement.

This consent is free of charge, not limited in terms of quantity, time, or territory. My image can be used for different forms of electronic image processing, editing, and composition, accepting the final product excluding offensive forms, generally considered to be unethical. My image cannot in any other way infringe my personal interests.

I hereby state that my image being processed for the purposes named above, linked to building the Expert Base of the Faculty of Management, Warsaw University, does not infringe my personal interests and I am aware that any time I have a right to inspect the material in which my image has been published.

I hereby confirm having read the information notice on personal data processing.
